Health services must report certain serious injuries, deaths, safety incidents and other data to Safer Care Victoria, the Victorian Agency for Health Information, or the independent councils we support.

Reporting information like this helps us understand where systems and processes are working well, and where there are opportunities to improve.

We share this data with health services to help them identify areas for improvement. We also release public reports.

Log all clinical, occupational health and safety incidents, as well as near misses and hazards in the Victorian Health Incident Management System (VHIMS)
Notify us of the most serious cases of patient harm and death that have resulted from adverse patient safety events
Report births, congenital anomalies and all perinatal, infant and child deaths
Report maternal deaths and serious harm to the Consultative Council on Obstetric and Paediatric Consultative Council (CCOPMM)
Report all surgical and anaesthesia-related harm and death to the Victorian Perioperative Consultative Council (VPCC)
Submit forms and permit applications under Victoria’s voluntary assisted dying laws
Victorian renal services submit data monthly for key performance indicators
