Timely recognition and response to frailty can improve health outcomes for older people and potentially reverse the degree of frailty. That’s why Safer Care Victoria’s Care of Older People Clinical Network is focusing on frailty as one of its quality improvement areas.
We’re looking for clinicians, academics and consumers to join our expert working group and help us better manage frailty in the primary care setting.
Why are we focusing on frailty?
Up to 27 per cent of older adults experience frailty and up to 50 per cent are pre-frail globally.
People who are frail are more likely to experience negative health outcomes, particularly when exposed to external factors.
Research shows that timely recognition and response can improve health outcomes. Our goal is to better develop a proactive, integrated and person-centred approach to frailty in the primary care setting.
As a member of our expert working group you will:
- contribute to a statewide quality improvement initiative to improve health outcomes for older people
- provide leadership and advice to SCV on the development and implementation of best practice approaches to recognising and responding to frailty
- share innovation and improvement across the Victorian healthcare system.
Who are we looking for?
Our expert working group is open to clinicians, academics and consumers with relevant expertise and experience in frailty.
