Introducing A Pathway to Phase II Cardiac Recovery, a cardiac rehabilitation program outline for clinicians. 
This new resource will help guide clinicians working in cardiac rehabilitation on what to include in their recovery program, according to the experts and latest evidence.
Developed by a National Heart Foundation and Deakin Institute Of Physical Activity & Nutrition (IPAN) collaboration, with funding from Safer Care Victoria’s Cardiac Clinical Network.
The pathway provides:

  • 40 best practice statements that services can prioritise as either essential or desirable 
  • example content
  • supporting links to guide clinicians on how to meet each recommendation.
