Safer Care Victoria is launching three cardiovascular projects to drive improvements in cardiovascular care.

Heart Failure Collaborative

In this collaborative, multidisciplinary teams will work intensively over 14 months to test and implement evidence-based change ideas to improve outcomes for heart failure patients.

Working with the Institute of Healthcare Improvement (IHI) hospitals will collaborate to drive improvements across the care continuum while building capability in improvement science.

This collaborative is open to more than 30 Victorian hospitals providing public or private acute cardiac care.

Cardiovascular ambassador project

Participating hospitals will receive funding for a dedicated cardiac liaison nurse to implement locally relevant models of care that reduce unplanned readmissions and improve the quality of cardiac care.  

Opportunities are available for up to 30 hospitals statewide to participate in this 18-month initiative with SCV facilitating professional development and networking opportunities. 

Please note: First preference in allocations will be assigned to hospitals who have also registered for the Heart Failure Collaborative.

Rapid access atrial fibrillation clinics pilot

Five regional health services will be funded for up to two years to establish atrial fibrillation clinics that improve access to timely evidence-based atrial fibrillation care and prevent avoidable strokes.  

This pilot is open to five regional Victorian health services providing acute cardiac care.
